IMMUNIZATION SCHEDULE
AND REQUIREMENTS

The following schedules indicate the recommended ages for routine administration of currently licensed vaccines for children. Any dose not administered at the recommended age should be administered at a subsequent visit when indicated and feasible.

0 – 6 YEARS:

7 – 18 YEARS:

  • Hepatitis A Vaccine: all students entering kindergarten, statewide, will be required to have 2 doses of Hepatitis A vaccine.
  • Measles, Mumps, Rubella (MMR) Vaccine: All students entering kindergarten, statewide, will be required to have 2 doses of Measles, Mumps, Rubella (MMR) vaccine.
  • Varicella Vaccine: All students entering kindergarten and 7th grade, statewide, will be required to have 2 doses of the varicella vaccine. A written statement from a parent (or legal guardian or managing conservator), school nurse, or physician attesting to a child’s positive history of varicella disease (chickenpox), or of varicella immunity, is acceptable in lieu of either dose of varicella vaccine.
